- Poor User input (mismatch of expectations)
- Lack of senior management/executive support
- Poor definition of project scope
- Unrealistic timescales
- Lack of, or inadequate, resources
- Incomplete or changing requirements and specification
- Lack of leadership and/or communication skills
- Lack of project specific skills/competence
- Poor stakeholder management
